- F137-A: Permanent Record
- Good Moral Character Certificate
- Photocopy of Birth Certificate from PSA
- Two pcs. 1x1 and two pcs. 2x2 colored pictures
- NCAE as may be required.
- 1 pcs. long brown envelope

For Transferees:
- Copy of Grades
- Honorable Dismissal from the previous school attended
- Good Moral Character Certificate
- Two pcs. 1x1 and two pcs. 2x2 colored pictures
